29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.  The employer does not ensure that scaffold legs bear on base plates and mud sills other adequate firm foundation.  This violation was observed on or about July 7, 2014 at the job site where employees were exposed to a fall hazard when working on a frame scaffold without base plates and mud sills.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffold legs bear on base plates and mud sills.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  The employer does not ensure that employees are protected from falling while accessing a scaffold.  This violation occurred on or about July 7, 2014 when employees were exposed to a fall hazard while doing masonry work from a scaffold without adequate access.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  The employer does not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by a fall protection system.  This violation occurred on or about July 7, 2014 when employees were exposed to a 10 foot fall hazard while performing stucco work on the second level of a residential building under construction without a fall protection system.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure employees engaged in residential construction activities at heights six feet or more are protected from falling.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.  The employer does not provide proper access to the employees where there is a break in elevation of 19 inches or more. This violation was observed at the construction site on July 7, 2014 where employees were exposed to a 10 ft. fall hazard when employees were ascending and descending the roof of a residential construction using the frame and cross braces of scaffolds.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provide with adequate access where there is a break in elevation of 19 inches or more.
