29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap. On or about July 3, 2014, on the west side of the residential home, employees engaged in cornice activities utilized an Makita skill saw with the power cord spliced exposing employees to electrical hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about July 3, 2014, on the west side of the construction site, one employee engaged in cornice activities from a Jack scaffold were exposed to falling more than 11 feet to the lower level. MR. JUAN MANUEL GUTIERREZ WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314273756, CITATION NUMBER 1, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 24, 2010, WITH AN FINAL ABATEMENT DATE OF JUNE 9, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 4199 STONE BRYER, MANSFIELD, TEXAS 75134.
