29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) At the establishment, in the granite wet sanding area, a metal electrical box having energized electrical wires connected by wire nuts and being used in a wet location did not have a cover exposing employees to an electrical shock hazard.

29 CFR 1910.305(j)(1)(iv): Fixtures installed in wet or damp locations shall be identified for the purpose and shall be so constructed or installed that water cannot enter or accumulate in wire ways, lamp holders, or other electrical parts.  a) At the establishment, in the granite wet sanding area where electrical outlets were exposed to water spray, a quad electrical outlet box, not designed for wet locations, was being used exposing employees to an electrical shock hazard.   b) At the establishment, in the granite wet sanding area where electrical outlets were exposed to water spray, three duplex electrical outlet boxes, not designed for wet locations, were being used exposing employees to an electrical shock hazard.   c) At the establishment, in the granite wet sanding area where electrical outlets were exposed to water spray, two single outlet electrical boxes, not designed for wet locations, were being used exposing employees to electrical shock hazards.  d) At the establishment, in the granite wet sanding area where electrical boxes were exposed to water spray, a metal electrical box having no cover and containing energized electrical wires connected by wire nuts was being used in a wet location exposing employees to an electrical shock hazard.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  a) At the establishment, a polisher, assigned to use a pneumatic sander to sand and polish granite slabs when making counter tops, had not received information and training on the hazards of silica dust, an inhalation hazard.
