29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. a) O'Neil Theater Job Site - On or about 7-8-14 - Employees were exposed to fall hazards from a pump jack scaffold system used to install siding on the exterior of the building which did not have the scaffold poles secured at the bottom.
