29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  (a) Carbide circular saw blade area- Employees were exposed to a struck-by hazard from compressed acetylene and oxygen gas cylinders that were not secured to prevent them from being knocked over. The cylinders were located beside a table and were not chained or strapped to anything for protection or stability.

29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition:  (a) Bandsaw and carbide circular saw blade areas-  Employees were exposed to skin irritation, sensitization and allergic contact dermatitis (skin rash), while cutting, welding, grinding and sharpening steel bandsaw and carbide circular saw blades containing metals such as, but not limited to, cobalt, manganese, copper, nickel, and chromium. The gloves employees were wearing, Superior Touch Dyneema, were not sanitary and/or reliable as indicated by traces of cobalt, manganese, and copper found on wipe samples taken from employees hands at the end of the shift.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  (a) Bandsaw and circular saw blade areas- The employer did not verify that a personal protective equipment workplace hazard assessment had been performed for employees using hazardous substances, such as but not limited to, cobalt, iron and manganese dust/fume generated during grinding and welding saw blades, Grind-Eze CE-150 containing naphthenic base oil, carbide grinding fluid containing sodium borate, and aluminum oxide generated from the grinding stone during the grinding operation.  The employer could not produce the written certification that identified the workplace evaluated, the person certifying that the evaluation had been performed, and the dates of the hazard assessment.
