29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   (a). Worksite located at 1820 N. Church Drive, Liberty, Missouri- Employees were exposed to overhead hazards from employees working on the roof structures and were not provided any means of head protection while exposed.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a). Worksite located at 1820 N. Church Road, Liberty, Missouri- Employees were exposed to fall hazards while working on a steep roof without the use of any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders' length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a). Worksite located at 1820 N. Church Road, Liberty, Missouri- Employees were exposed to fall hazards in that 2 separate ladders being used on the jobsite did not extend at least 3 feet above the upper landing surface. One ladder was used to access the main roof level, while the other ladder was used to access the upper roof from the lower roof.
