Warehouse - On or about June 26, 2014, the employer failed to provide refresher training to a forklift operator after a near miss incident, thereby exposing employees to serious multiple injuries.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. Throughout Facility - On or about June 9, 2014, the employer failed to ensure that a powered industrial vehicle operator slow down and sound the horn at cross aisles and other locations where vision is obstructed, exposing employees to struck by hazards.
