29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:  a) In the shop area - The employer failed to ensure that employees were equipped with foot protection when working with tools, welding supplies and fire extinguishers. Employees were exposed to foot injuries due to objects piercing the sole and falling objects.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) In the shop area - The employer failed to securely mount and identify the portable fire extinguishers employees are required to use in the event of a fire.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) In the shop area - The employer did not provide training to employees for hazard communication program and also for information in new versions of Revised 1910.1200 for Globally Harmonized system (GHS) of classification and labeling of chemicals for employees working with water coolant fluid called Cimcool fluid for grinders. Employees were exposed to skin and eye irritation.
