29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On the site beside the concrete mixer, the platform used for employees to step onto the ladder to the mortar storage bins, there was a hole which measured 40 inches by 6 inches thereby exposing employees to a trip/fall hazard.
