29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Independent Roofing Specialists, LLC at 251 W Diamond, Granby, CO 80446: On July 9, 2014 the exposing employer did not ensure employees were protected from injury, in that employees were working at the edge of a 4-in-12 pitch roof without the use of a fall protection system. This condition exposed employees to a fall of approximately 11 feet.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: (a) Independent Roofing Specialists, LLC at 251 W Diamond, Granby, CO 80446: On July 9, 2014 the employer allowed employees to use a stepladder to access the roof of the building from the ground level. The stepladder was used contrary to manufacturer's instructions, in that the front and back section of the stepladder was not separated and locked as required by the manufacturer. This condition exposed employees to a fall hazard of approximately 11 feet.
