29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary:   a)   359 South White Horse Pike:      Employees used the 24 foot Werner fiberglass extension ladder extended approximately 17 inches above the upper surface being accessed to get onto the roof, on or about 07/02/12;  b)   359 South White Horse Pike:      Employee stood on top of the and on the top step of the yellow 8 foot step ladder while attaching trim under the eave,  on or about 07/03/14. 29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:      a))   359 South White Horse Pike:    Employees were using pneumatic nail guns, applying roofing felt, shingles, and flashing without any eye protection, on or about 07/02/14;      b)    359 South White Horse Pike:    Employees without any eye protection were working under the edge of the roof installing trim for the fascia and soffit, on or about 07/02/14;       c)    359 South White Horse Pike:    Employees were using pneumatic nail guns, applying roofing felt, shingles, and flashing without any eye protection, on or about 07/02/14;         d)    359 South White Horse Pike:    Employees without any head protection were working under the edge of the roof installing trim for the fascia and soffit, on or about 07/03/14.
