29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. CO2 room; on or about 08 July 2014 and times prior thereto; the eyewash station was not inspected and maintained in accordance to manufacturers requirements for quick drenching or flushing of the eyes. The protective caps were not installed. Last recorded date of inspection was April 2014. When tested rust colored water flowed from the fountain. This condition exposed employees to eye trauma hazards.

29 CFR 1910.1200(f)(5): Chemical manufacturers, importers, or distributors did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged, or marked in accordance with 29 CFR 1910.1200. CO2 room; on or about 08 July 2014 and times prior thereto; an unmarked plastic spray bottle, containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns and skin dermatitis.
