29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section:    a) On July 10, 2014 specific, written procedural steps for the control of all potential sources of hazardous energy were not developed and documented.  Documentation on the identification and location of energy sources, process of installing, removing, transferring and testing the lockout / tag-out devices for their effectiveness was not available to employees who perform scheduled and unscheduled maintenance on machinery and equipment.  Maintenance activities include, but are not limited to, band saw changes on the head rig, and blade changes on the re-saw.        Empire Lumber Company, Weippe Operations was previously cited at their sister location, Empire Lumber Company dba Kamiah Mills, for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 313354599, citation number 1, item number 1 and was affirmed as a final order on August 17, 2010, with respect to a workplace located at Hwy 12, Railroad Street, Kamiah, ID 83536.     Note) Certification of corrective action and documentation ARE required for this item.
