29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control a) Mitsubishi 40" printing press; On or about July 10, 2014 and at times prior thereto, the written machine specific lock-out tag-out procedure did not contain the specific requirements for testing the machine or equipment to determine and verify the effectiveness of the lockout devices for the 120 VAC electrical and 120 psi pneumatic energies that were present on the machine. b) Sakurai 20" x 26" printing press; On or about July 10, 2014 and at times prior thereto, the written machine specific lock-out tag-out procedure did not contain the specific requirements for testing the machine or equipment to determine and verify the effectiveness of the lockout devices for the 120 VAC electrical and 120 psi pneumatic energies that were present on the machine.
