29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control  a)  Mitsubishi 40" printing press;  On or about July 10, 2014 and at times prior thereto, the written machine specific lock-out tag-out procedure did not contain the specific requirements for testing the machine or equipment to determine and verify the effectiveness of the lockout devices for the 120 VAC electrical and 120 psi pneumatic energies that were present on the machine.  b)  Sakurai 20" x 26" printing press;  On or about July 10, 2014 and at times prior thereto, the written machine specific lock-out tag-out procedure did not contain the specific requirements for testing the machine or equipment to determine and verify the effectiveness of the lockout devices for the 120 VAC electrical and 120 psi pneumatic energies that were present on the machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a)  Mitsubishi 40" printing press;  On or about July 10, 2014 and at times prior thereto, the employer did not perform an annual periodic inspection of the energy control procedures with authorized employees that have the potential to perform lock-out tag-out (maintenance) on the Mitsubishi printing press.  b)  Sakurai 20" x 26" printing press;  On or about July 10, 2014 and at times prior thereto, the employer did not perform an annual periodic inspection of the energy control procedures with authorized employees that have the potential to perform lock-out tag-out (maintenance) on the Sakurai printing press.
