29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections: On or about July 3, 2014 and at times prior thereto two employees were using two ladders tied together, on the side of the two story residence, to gain access and egress from a roof. This condition exposed the employees to a fall hazards of approximately 22-feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: On or about July3, 2014 and at times prior thereto two employees were using two ladders tied together, on the side of a two story residence, to gain access and egress from the roof; the employer did not provide ladder training to enable employees to recognize the hazards. Hazards associated with roofing such as but not limited to falls and/or struck-by hazards.
