Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) On or about 7/10/2014, on the west side of the building, employees working on a flat roof without fall protection were exposed to falls of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) On or about 7/10/2014, on the west side of the building, employees used a ten-foot Husky step ladder by leaning it against the wall to access the higher roof level. The employees were exposed to fall hazards of approximately 8 feet to the lower level of the roof.
