29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    On or about July 9, 2014, on the north side of the residential home, employees engaged in masonry activities were not wearing hardhats while working below other employees.  The employees were exposed to struck-by flying and falling objects.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.   On or about July 9, 2014, on the north side of the work site, employees using a MP1 mason saw with the power cord outer sheathing pulled away exposing the internal wiring were exposed to electrical hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access.   On or about July 9, 2014, on the north side of the construction site, employees engaged in masonry activities from a tubular welded frame scaffold were not provided a ladder to access the upper and lower levels exposing the employees to an unprotected fall hazard of more than 13 feet to the lower level.
