29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At this job site, employees were not furnished a safe means of egress from a trench approximately 5.3 feet deep. The employees were stepping on an installed PVC pipe and then stepping onto the slight bank of the trench to egress from the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that: At this construction site ., employees were exposed to trench collapse hazards while conducting pipe laying operations. Employees entered and worked in a trench greater than 5 feet deep in which the bench method of employee protection was used but did not extend to the area where employees were observed working.
