29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  Tulapai Flats: On or about June 19, 2014 and at times prior thereto, the electric cord set used had damaged insulation exposing employees to an electrical hazard.  The cord was used to provide power to the Magic Chef microwave and used weekly on worksites.
