29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a)  There were several unprotected floor openings at the top deck. Employees were exposed to falling 10 feet to the floor below.    Location: Construction site; 182 W 82nd Street, New York, N.Y.    On or about 7/9/2014    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with:  (i)  At least one handrail; and   (ii)  One stairrail system along each unprotected side or edge.    a)  Several runs of the stairway did not have handrails or stair-rails. Employees were exposed to trip/ fall hazard when using the stairs during construction activities.    Location: Construction site; 182 W 82nd Street, New York, N.Y. 29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least three feet above the upper landing surface; or, when such an extension is not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device such as a grabrail was not provided:    a) There was an extension ladder being used by construction personnel to get from 3fl to 4fl.  The ladder was not secured at the top and extended only 1 ft above the upper landing. Employees were exposed to 10 ft fall hazard when getting on and off the top of the ladder.    Location: Construction site; 182 W 82nd Street, New York, N.Y.
