29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.  (a) Rooftop relieve valves: On or about July 7, 2014 and at times prior thereto, the employer did not document that relief venting for equipment complied with RAGAGEP in that, the relief vents were not 15 feet about the adjacent grade or roof level. Ammonia equipment includes, but is not limited to, EV-1; EV-2 and EV-3.  (b) Site ammonia system: On or about July 7, 2014 and at times prior thereto, the employer did not document that ammonia refrigeration system piping and components were properly identified. Ammonia equipment with missing tags includes, but is not limited to, the HX-1 (three valves) and EV-1; EV-2 and EV3.  (c) Site ammonia system: On or about July 7, 2014 and at times prior thereto, the employer did not document that the ceiling and door openings for the machinery room had tight fittings to the outside. The ceiling/wall opening were visible; the overhead door on the south end of the machine room, and man doors on the SW and SE sides of the room were blocked open by water hoses. 29 CFR 1910.119(n): The employer did not establish and implement an emergency plan for the entire plant in accordance with the provisions of 29 CFR 1910.38:  (a) Gooding Site main office: On or about July 7, 2014 and at times prior thereto, from approximately 1:30-1:50 pm, emergency radios were not activated in response to a power outage.   (b) Gooding Site main office: On or about July 7, 2014 and at times prior thereto, emergency exit lighting was not illuminated with emergency power in response to power outage.    (c) Gooding Site facility, Emergency Action Plan: On or about July 7, 2014 and at times prior thereto, the emergency responder list had not been kept up to date.  (d) Gooding Site facility, Emergency Action Plan: On or about July 7, 2014 and at times prior thereto, the ammonia monitoring action levels were not consistent throughout the plan, nor were they linked to the evacuation levels.
