29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Employees using a pneumatic nail gun during construction activities on a roof were exposed to eye injuries and the employer did not ensure employees wore suitable eye protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     Employees performing construction activities on a roof were exposed to 11.5 foot fall hazards and the employer did not ensure employees used a suitable fall protection system and developed and implemented a fall protection plan.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees performing construction activities on a roof were exposed to 11.5 foot fall hazards and the employer did not ensure employees were provided fall protection safety training in order to provide them with the knowledge necessary to recognize the hazards, address the requirements associated with fall protection and describe the proper work procedures to follow to minimize or eliminate the fall hazards.
