29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation. On or about July 10, 2014, and at times prior thereto, the employer did not ensure that each power industrial truck operator at the warehouse has successfully completed the training required by 1910.178(1) paragraph (l). This condition exposed employees to struck-by and caught-in between hazards.
