29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about July 9, 2014, Kyle Parkway, Kyle, TX, the employer failed to ensure an employee was protected from a fall hazard. Employee was observed working from an aerial basket without fall protection. The employee was welding slanted J channel. Employee was exposed to a fall hazard of approximately 7 feet.
