29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 385 Windsor Street, North Plainfield, NJ 07060: Employee was observed performing ground operations, with overhead roofing work being performed, without a protective helmet.  Employee was exposed to possible danger of head injury from impact with falling or flying objects.    Condition observed on or about July 11, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:     a) 385 Windsor Street, North Plainfield, NJ 07060: Employees were observed performing roofing operations at a height of approximately 12'4" to the ground from the eave with no fall protection in place, exposing them to fall hazards.    Condition observed on or about July 11, 2014.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a) 385 Windsor Street, North Plainfield, NJ 07060: Employees were allowed to use ladders that were not secured and did not extend 3 feet above the working surface to allow for safe access on and off the roof.       Condition observed on or about July 11, 2014.
