OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees who were exposed to falling and crushing hazards related to the operation and maintenance of under hung cranes:    a) Shop - Management did not provide training for employees in the safe operation of under hung cranes and did not administer written and practical examinations intended to verify that the person has acquired the knowledge and skill to operate the under hung cranes. The employer did not issue a certificate, or formal record, that verified that the person had been trained and had passed the written and practical examination as required by ASME B30.11 section 11-3, "Operator Training and Operation".     b) Crane #2 Space Master 2000 under hung wire rope crane - The employer did not ensure that a warning tag was in place at the pendant control in accordance with ASME B30.11-1.1.5.    c) Crane #3  Acco 5 Ton bridge with a wire rope Wright 5 ton under hung crane (Paint Area) - The employer did not ensure that the pendant directional markings were legible in accordance with ASME B30.11-1.1.6(a). The directional control markings were painted over and not readable.    d) Crane #4 Space Master 2000 under hung wire rope crane - The employer did not ensure that the pendant directional markings were legible in accordance with ASME B30.11-1.1.6(a). The pendant control markings for "up", "down", and "east" were missing.       e) One ton Acco Wright under hung jib crane in side shop - The employer did not ensure that the safety latch was operational in accordance with ASME B30.11-2.1.4. The safety latch for the hook was sprung out of the hook.     f) One ton Acco Wright under hung jib crane in side shop - The employer did not ensure that the pendant directional markings were legible in accordance with ASME B30.11-1.1.6(a). There were no directional markings on the pendant controller.       Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    a) Welding Shop - Employees lift and position structural steel I beams with under-hung cranes, fabricating custom specification structural elements for buildings using cutting, welding and painting processes and are exposed to multiple hazards of industrial fabrication.  The employer has not prepared a "Certification of Hazard Assessment", document describing the expected hazards identified in the shop and required personal protective equipment required for each assigned task, on or about July 11, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
