(a) On Park Crest Drive (near the intersection of Park Crest Drive and Sybil Court), on or about 07/14//2014, the employer failed to provide adequate signage and traffic cones needed for work with a lane closure on a low-volume two-lane road, exposing employees to a potential struck-by hazard from oncoming traffic (two lane traffic, one lane for each direction.)
