29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a) At the intersection of Park Crest Drive and Sybil Court, St. Louis, MO, on or about 07/14/2014, an employee of T. Drury Contracting, Inc. was working in a trench where spoil piles had been placed near the edge of the trench, exposing himself to a potential struck by hazard from falling debris.

(a) At the intersection of Park Crest Drive and Sybil Court, St. Louis, MO, on or about 07/14/2014, an employee was exposed to a struck by hazard when he was working in an area of trench greater than five feet in depth that had spoil piles and shale positioned by the trench edge (less than 2 feet away from the edge of the trench.)

(a) On Park Crest Drive (near the intersection of Park Crest Drive and Sybil Court), on or about 07/14//2014, the employer failed to provide adequate signage and traffic cones needed for work with a lane closure on a low-volume two-lane road, exposing employees to a potential struck-by hazard from oncoming traffic (two lane traffic, one lane for each direction.)
