29 CFR 1910.254(d)(8): Welding cables with splices within 10 feet (3 m) of the holder were used:  On or about July 14, 2014, in the front shop and middle shop the employer failed to ensure that welding leads were not in damaged condition.   Instance 1) In the front shop employees were using an arc welding machine that had damaged leads. There were 3 areas with tape around them. The damage areas are between 2' and 7'9" from the holder.    Instance 2) In the front shop employees were using an arc welding machine that had damaged leads.  There were 5 damaged areas with tape around them. The damaged areas were at 1'8", 3', 4', 4'5", and 9' from the holder.   Instance 3) In the middle shop employees were using an arc welding machine that had damaged leads. There were 3 damaged areas with tape around them. The damage was located at 1, 2, and 4 from the holder.
