29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on equipment (such as compressors, mixers, screens, or pumps used for concrete and masonry construction activities) where the accidental start-up of the equipment was not eliminated by use of lock out/tag out procedures: a) At the concrete plant, located at 16120 Hwy. 6, Rosharon, Texas, on or about July 15, 2014, employees were exposed to caught-in hazards while performing maintenance and clean-up work inside a Mixer Drum on a Vance Hagan Portable High Production Horizontal Mixer without locking out all sources of energy, including but not limited to, electrical, hydraulic, and pneumatic energy.
