29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a)      On or about 07-10-14, employees who were using a Gehl DL-6H 40 Dynalift Telescopic Handler had attached a personnel platform to the forks and elevated personnel to heights approximately fifteen (15) feet and twenty-five (25) feet above compacted surface without manufacturer's approval.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a)      On or about 07-10-14, employees who were accessing/egressing a scaffold platform to work near a rectangular window were walking on a garage roof surface at fourteen and one half (14.5) feet above compacted surface without utilizing fall protection equipment.    b)      On or about 07-10-14, employees who were accessing/egressing a scaffold platform to work near a round window were walking on a house roof surface at approximately twenty-five (25) feet above compacted surface without utilizing fall protection equipment.
