OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical burns and inhalation hazards from ammonia:    Ammonia refrigeration system: The machinery room was not provided with approved self closing labeled fire doors, restricted access, visual/audible alarms outside of each entrance door and emergency remote controls for ventilation and to stop the action of the refrigerating compressors immediately outside the machinery room, on or about July 16, 2014.    Ammonia refrigeration system: All piping mains, headers and branches throughout the facility were not identified as to the physical state of the refrigerant (vapor, liquid, etc.), on or about July 16, 2014.    Abatement Certification Required.
