29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.    On or about 16 July 2014, and at times prior thereto, the employer allowed various items to be placed on an upper mezzanine level that served as the ceiling of an office area underneath without knowing the load capacity of the ceiling, exposing the employees working in the office to the hazard of being struck by falling beams and various other items in storage.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    On or about 16 July 2014, and at times prior thereto, employees accessed an upper storage area to remove or store various items that did not have guardrails installed, exposing the employees to a fall hazard of approximately 9 feet 3 inches to the concrete floor below.

29 CFR 1910.25(d)(2)(xii): Tops of the ordinary types of stepladders were used as steps.  On or about 16 July 2014, and at times prior thereto, employees would use a portable stepladder to access the upper mezzanine level that served as extra storage space by using the top step, exposing the employees to a fall hazard of approximately 9 feet 3 inches to the concrete floor below.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years.    On or about 16 July 2014, and at times prior thereto, two employees who utilize the forklift had not been retrained within the last three years to determine their operating skills and validate their being able to operate the forklift, exposing them and those employees working around them to the hazard of being struck by the forklift.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.    On or about 16 July 2014, and at times prior thereto, the employer allowed the use of three electrical outlets that had missing or damaged faceplates with exposed energized parts, exposing the employees using them to an electrical hazard of approximately 120 volts.  1) An energized four plug outlet located along the east wall of the warehouse had a damaged faceplate that exposed the internal wires and terminals.  2) An energized four plug outlet located along the north wall of the warehouse had a missing faceplate that exposed the internal wires and terminals.  3) An energized four plug outlet located along the north wall of the warehouse had a damaged faceplate that exposed the internal wires and terminals.

29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.  On or about 16 July 2014, and at times prior thereto, employees used two light switches to turn on or off the warehouse lights on a daily basis in which the faceplate was missing for both switches, exposing the employees to the electrical hazard of approximately 120 volts.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.     On or about 16 July 2014, and at times prior thereto, an orange extension cord was being used as fixed wiring within the office area to provide power to a copier machine in which the cord was nailed to the wall with brackets, exposing the employees within the office to a fire hazard due to cord damage.
