29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) R.G. Smith of Mansfield, Inc. on or about July 15, 2014, at the address of 715 Richland Mall, Mansfield, OH: The employer failed to ensure employees landing and placing boiler/chiller units though the 2nd story unguarded mechanical opening were adequately protected from falls. The employees working in and around this condition were exposed to a near 16 ft. fall to the ground below.
