29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  a)  129 Midland Ave, Garfield, NJ: CSHO observed an employee exposed to fall hazards of approximately 12 feet as the scaffold work platform was not fully planked, on or about 07/15/2014.   Galindo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 930349 citation number 1, item number 1 and was affirmed as a final order on 12/20/2013, with respect to a workplace located at 658 Route 17 North, Paramus, NJ.  Galindo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1),  which was contained in OSHA inspection number 935604, citation number 1, item number 1 and was affirmed as a final order on 4/01/2014, with respect to a workplace located at 776 Route 17 North, Paramus, NJ.
