29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: The employer does not protect employees engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels from falling by guardrail systems, safety net systems, personal fall arrest system. This violation was observed on or about July 16, 2014 on the north side of the roof where employees were exposed to a fall hazard greater than 6 feet when blowing leaves off a roof without using fall protection. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 6 feet or more above a lower level or ground.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. The employer does not ensure that portable ladders with structural defects, or defective components are removed from service until repaired. This violation was observed on or about July 16, 2014, on the north east side of the building where employees were exposed to a fall hazard when using a fiberglass extension ladder that had two bent swivel safety shoes and a broken rung support, and had not been taken out of service. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure portable ladders with structural defects, corroded components or other faulty components are immediately withdrawn from service until repaired.
