29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) On or about July 14th, 2014, at a construction site located at 2427 Mid American Drive in Boonville, MO, two employees were exposed to a fall hazard of greater than 6 feet while working on a roof without any type of fall protection system in place or in use on the day of the inspection.    Abatement certification for this Item is not required.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:   (a) On or about July 14th, 2014, at a construction site located at 2427 Mid American Drive in Boonville, MO, 3 employees were exposed to potential fall hazards of greater than 6 feet (1.8m) while working around a second story wall (window) opening where the inside bottom edge of the wall opening was less than 39 inches (1.0m) above the walking/working surface.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
