29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. (a) Guard(s) on the F.L Smithe RA - 2 envelope folding machine were not affixed to the machine. The panel door located below the envelope infeed was not secured in a closed position and could be opened while the machine was in operation, exposing employees to amputation and caught-in-between hazards.

Employees were able to access the point of operation created by the ingoing rollers, exposing employees to amputation, crushing, and caught-in-between hazards.
