29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: 29 CFR 1926.403(b)(1)(iii): Electrical insulation. a. On the second floor of the parking garage where ballast were installed in front of the elevators, the electrical extension cord was cut through the insulation exposing copper wire thereby exposing employees to a contact hazard.

On the second floor of the parking garage where ballasts were installed near the entrances of the building, the extension cord used to provide power to the portable power tool was missing a ground pin thereby exposing employees to a contact hazard.
