29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. On or about July 15, 2014, the employer did not ensure the point of operation of the Sterling, Accumulator Type Molding Press #7 was guarded. The opening between the conveyer belt below the press and the bottom of the door guard was approximately 21 inches high by 69 inches wide. b. On or about July 15, 2014, the employer did not ensure the point of operation of the Sterling, Accumulator Type Molding Press #8 was guarded. The opening between the floor and the bottom of the door guard was approximately 39 inches high by 6 feet wide. The door guard is approximately 34 inches from the point of operation. c. On or about July 15, 2014, the employer did not ensure the point of operation of the Sterling, Accumulator Type Molding Press #13 was guarded. The opening between the floor and the bottom of the door guard was approximately 32 inches high by 5 feet wide.
