29 CFR 1910.23(e)(1): A standard railing did not consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level:    a/  Inside of the Fabrication Shop, the stairrail belonging to a metal flight of stairs consisting of 14 risers was missing a mid-rail.    b/  Inside of the Receiving Department, two, metal flights of stairs consisting of five risers were missing a standard railing.

The specific P.O.O. include the punch and flat shearing station.
