Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     (a) On or about July 12, 2014, and at times prior to this date, for employees exposed to an approximate 55-foot fall hazard while working from the tubing board during tripping out operation where the employer had installed a Geronimo emergency escape device anchor approximately 80 feet from the well bore at Amber Renee 25-36H, Sec. 25 T154N R91W, Stanley, ND.        (b) On or about July 12, 2014, and at times prior to this date, for employees exposed to an approximate 55-foot fall hazard while working from the tubing board during tripping out operation where the employer had installed a Geronimo emergency escape device cable with one cable clamp on the ground anchor at Amber Renee 25-36H, Sec. 25 T154N R91W, Stanley, ND.         Abatement Note: Among other methods, one feasible and acceptable means of abatement to correct the hazards would be for the employer to correctly install the Geronimo emergency escape device cable length out to a minimum distance of twice the height of the work platform and to ensure the correct cable clamp orientation of at least three API approved cable clamps in accordance with the Manufacturer's Operators Manual, pages 11 - 13, INSTALLATION, Illustration 3 and 4. Also the (API) American Petroleum Institute Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), section 6.10, AUXILIARY ESCAPE, subsection 6.10.1a, "It is recommended that the ground anchor point of the escape line should be located a minimum lateral distance from the derrick or mast equal to two times the height of the work platform."

Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:     (a) On or about July 12, 2014, and at times prior to this date, for employees exposed to a stuck-by hazard while utilizing the rig platform stairway where a high-pressurized Kelly hose was not secured at Amber Renee 25-36H, Sec. 25 T154N R91W, Stanley, ND.        Abatement Note: Among other methods, one feasible and acceptable means of abatement to correct the hazard would be for the employer to ensure flexible high-pressure hoses are secured by an acceptable means in order to maintain control of the hoses in case of a failure in accordance with (API) American Petroleum Institute Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), section 9.13, PRESSURE EQUIPMENT, subsection 9.13.3.
