29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Strongsville Hardware Co., Inc. dba True Value Hardware:  On or about July 10, 2014, the employer did not ensure that employees successfully completed an operational training program prior to permitting employees from operating a powered industrial truck.  An employee used a sit-down counterbalance lift truck to lift and/or maneuver pallets of product outside of the facility.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     a) Strongsville Hardware Co., Inc. dba True Value Hardware:  On or about July 16, 2014, employees were exposed to electric shock and fire hazards in that flexible cords and a three way power adaptor were not being used as listed.  The cords and adaptor were in a series to power multiple florescent lamps in the back room over the glass cutting table.

29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipement grounding connection were used:     a) Strongsville Hardware Co., Inc. dba True Value Hardware:  On or about July 16, 2014, employees were exposed to electrical shock hazards, in that a three-prong to two-prong adaptor was used on the pipe cutting machine, interrupting the continuity of the equipment grounding connection.
