An employee used a sit-down counterbalance lift truck to lift and/or maneuver pallets of product outside of the facility.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Strongsville Hardware Co., Inc. dba True Value Hardware: On about July 16, 2014, the employer did not ensure that employees were protected from hazards associated with the Ridge manufactured pipe cutter. The rotating chuck of the pipe cutter was not guarded to protect employees from getting caught in the jaws.

The cords and adaptor were in a series to power multiple florescent lamps in the back room over the glass cutting table.
