Metal storage bins (walk-in type) located in the yard and metal racks located in the Shop and in the yard are used to store metal and other items that are used on/off site.  Aisles were not maintained free of storage material exposing employees to sharp metal edges and tripping hazard.   Observed on/or about 07/16/2014.

Paint Finishing Area: Sunnyside Pure Gum Turpentine/Thinner (Flash Point 91 F/ Boiling point 302 F) Class 1C Category III flammable liquid.    Turpentine/Thinner (including but not limited to) is stored in and dispensed from 1 gallon metal containers without the use of a safety can or an approved self-closing valve.  Observed on/or about 07/16/2014.

Yale Forklift Truck.  The eye wash facility was not immediately available for use in that the water line to the plumbing type eye wash was broken.  Observed on/or about 07/16/2014                                             Propak/43 Stults Rd. Site: Warehouse.  An eye wash facility was not immediately available for use where employees are required to check powered industrial truck battery fluid levels on a routine basis.    Observed on/or about 05/01/2014

Two forklift operators who were alleged to have been previously trained were not evaluated relative to the forklift training at least every 3 years.  The last training was alleged to have been conducted approx. 6 years ago but no documentation of that training was available for review.   Observed on/or about 07/16/2014.
