29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) SHINGLE CUTTING DEPARTMENT, WEST END - Employees changed blades on Band Saw A (S/N 8646) where the authorized employee de-energized the saw using the power disconnect switch on the South wall and the breaker panel on the North wall, but did not apply any locking device to the disconnect or the breaker, thereby exposing employees to serious injury, on or about July 17, 2014. (b) SHINGLE CUTTING DEPARTMENT, MIDDLE - Employees changed blades on Band Saw B where the authorized employee de-energized the saw using the power disconnect switch on the South wall and the breaker panel on the North wall, but did not apply any locking device to the disconnect or the breaker, thereby exposing employees to serious injury, on or about July 17, 2014. (c) SHINGLE CUTTING DEPARTMENT, MIDDLE - Employees changed blades on Band Saw C where the authorized employee de-energized the saw using the power disconnect switch on the South wall and the breaker panel on the North wall, but did not apply any locking device to the disconnect or the breaker, thereby exposing employees to serious injury, on or about July 17, 2014. 29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) THE TUNNEL SHINGLE CUTTING DEPARTMENT - Employees cut shingles using a Wilder 1624 Workhorse Slitter Machine (SN 39196, Asset Tag 30-109), and the cutting wheels were not fully guarded, thereby exposing employees to the in-running nip points of the cutting wheels, on or about July 17, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(b) THE TUNNEL, SHINGLE CUTTING DEPARTMENT - Employees used band-saw B to cut shingles, and a portion of the blade measured to be approximately 7 inches long was not guarded from contact, thereby exposing employees to amputation hazards, on or about July 17, 2014. (c) THE TUNNEL, SHINGLE CUTTING DEPARTMENT - Employees used band-saw C to cut shingles, and a portion of the blade measured to be approximately 5.5 inches long was not guarded from contact, thereby exposing employees to amputation hazards, on or about July 17, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
