29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone:    Instances including, but not limited to the following:    a) Jobsite, 9 Clairton Boulevard, Pittsburgh ,PA: On or about July 17, 2014, during roofing work on a low sloped roof, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to falls of up to approximately 50 feet. On the main roof, the employer relied on parapet walls that were too short to serve as fall protection along 3 sides of the roof, and had a warning line set up along the rear of the roof that was located on the edge of the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enable each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards:   a) Jobsite, 9 Clairton Boulevard, Pittsburgh ,PA: On or about July 17, 2014, during roofing work on a low sloped roof, the employer did not ensure that employees received fall protection training where exposed to falls of up to approximately 50 feet.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed:   a) Jobsite, 9 Clairton Boulevard, Pittsburgh ,PA: On or about July 17, 2014, the employer did not ensure that ladders were used only for the purpose that they were designed for in that to access a lower area roof, the employer set up a stepladder that was folded up and leaned against the building.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize these hazards:   a) Jobsite, 9 Clairton Boulevard, Pittsburgh ,PA: On or about July 17, 2014, the employer did not ensure that ladder safety training was provided to all employees. The inspection revealed that the employer set up a stepladder that was folded up and leaned against the building and used it for access to a lower level roof.
