29 CFR 1910.38(c)(1): The employer failed to develop and implement procedures in their emergency Action Plan for reporting a fire or other emergency:     On or about July 16, 2014, the employers emergency action plan contained inadequate procedures for reporting a grain storage bin emergency.  Employees were exposed to aggravated injury from delayed response by first responders.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about July 16, 2014, an employee was exposed to caught-in and/or amputation from mechanical hazards while working within grain bin T-10 where an auger was operated. The auger was not disconnected, locked out and tagged out.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
