29 CFR 1910.36(c)(1): Each exit discharge did not lead directly outside or to a street, walkway, refuge area, public way, or open space with access to the outside.  a) In the Warehouse department - The employer failed to provide each exit discharge with clean access to outside the building when the overgrowth of vegetation covered the exit discharge. Employees were exposed to fire hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:   a)Throughout the facility  - The employer failed to provide each portable fire extinguisher with the signs for employees to identify and have access easily when working with tires and wheels and operating the propane forklifts. Employees were exposed to fire hazards.

29 CFR 1910.1200(f)(5)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings:   a) In the Foam Fill department - The employer did not provide any label for information regarding the hazards of chemicals when employees were working with a container of "PART C" that included Cured Tire Flatproofing Elastomer with Flammability Class IIIB, a combustible liquid, exposing employees to eye and skin irritations and fire hazards.

29 CFR 1910.179(j)(1)(ii): The employer did not perform frequent and periodic inspections as defined in paragraphs (a) and (b) of this section:  a) In the Warehouse department  - The employer failed to maintain the inspections for the hoist chains including visual daily inspection, monthly inspection and signature of the person that performed the inspection when employees were operating Dayton Electric Chain Hoist. Employees were exposed to head injuries and head trauma.
