MCM Precision Castings, Inc. - Weston, Ohio: On August 20, 2014, the employer did not ensure that the energy isolating device that is needed to control energy to the Arnold Machine Co. 4 belt grinder was operated so as to isolate energy while employees serviced the machine.  The grinder was not locked out when belts were changed.      b. MCM Precision Castings, Inc. - Weston, Ohio: On or about November 3, 2014, the employer did not ensure that the energy isolating device that is needed to control energy to the cut-off saw was operated so as to isolate energy while employees serviced the machine.  The cut-off saw was not locked out when the blade was changed.

MCM Precision Castings, Inc. - Weston, Ohio: On or about July 17, 2014, the employer did not ensure that the Arnold Machine Co. 4 belt grinder was adequately guarded to protect employees from hazards associated with rotating parts.  The spindle end and nut on the right facing side of the grinder was not guarded.    b. MCM Precision Castings, Inc. - Weston, Ohio: On or about August 20, 2014, employees that operated the Burr King belt sander (model 760) were not protected from hazards created by the rotating parts.  The employer did not assure that the ingoing nip-point located between the belt and bottom wheel was guarded.      c. MCM Precision Castings, Inc. - Weston, Ohio: On or about August 20, 2014, employees that operated the Kalamazoo belt sander (model 5272) were not protected from hazards created by the rotating parts.  The employer did not assure that the ingoing nip-point located between the belt and front wheel was guarded.      d. MCM Precision Castings, Inc. - Weston, Ohio: On or about August 20, 2014, the employer did not ensure that the small wax press located in the wax room was guarded to protect employees from pinch points created when the machines hold downs operate to clamp the die blocks.

MCM Precision Castings, Inc. - Weston, Ohio: On or about August 18, 2014, the employer did not ensure that employees who perform electrical tests to troubleshoot electrical equipment were provided with and used personal protective equipment to protect against electrical hazards.  An employee using a voltage detector to troubleshoot fuses at a 480 volt distribution box for the shot blaster was exposed to electrical shock and arc flash hazards in that flame resistant clothing, insulated gloves with leather gloves, hard hat, and arc rated face shield were not worn during the troubleshooting activities.
