29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer does not provide a safe means of egress from excavations. This violation was observed on or about July 18, 2014, at the job site where employees were exposed to a fall hazard when exiting the excavation without a safe means of egress. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provided a safe means of egress are provided in excavations 4 feet or more in depth.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): The employer does not protect employees from excavation cave-ins by an adequate protection system. This violation was observed on or about July 18, 2014, at the job site where employees were exposed to a crushed-by hazard when working in an excavation without being protected from cave-ins by an adequate protection system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from crushed-by injuries form cave-ins while working in excavations.
