29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:     This violation occurred on or about July 18, 2014 at a jobsite located at 508 Park Avenue in Dallas, Texas 75201; where at least four (4) employees were working on the second level of a tubular welded frame scaffold whose platform was not fully planked and had gaps larger than 1 inch between adjacent planks. The employees were not protected from falling approximately 13 feet to the ground below.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights were not plumb and braced to prevent swaying and displacement:    This violation occurred on or about July 18, 2014 at a jobsite located at 508 Park Avenue in Dallas, Texas 75201; where at least four (4) employees were working on the second level of a tubular welded frame scaffold whose frames were not plumb and braced to prevent swaying and displacement. The employees were not protected from falling approximately 13 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold end frames were used as a means of access.    This violation occurred on or about July 18, 2014 at a jobsite located at 508 Park Avenue in Dallas, Texas 75201; where at least four (4) employees were working on the second level of a tubular welded frame scaffold and were accessing the scaffold by climbing the end frames. The employees were exposed to falling approximately 13 feet to the ground below.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):    This violation occurred on or about July 18, 2014 at a jobsite located at 508 Park Avenue in Dallas, Texas 75201; where at least four (4) employees were working on the second level of a tubular welded frame scaffold and did not have a guardrail system or personal fall arrest system in place. Employees were exposed to falling from the open ends of the scaffold to approximately 13 feet to the ground below.
